Transplant Australia’s Medical Committee will manage the needs of athletes and their families at the Games.
Medical Forms
Transplant recipients are required to obtain medical clearance from their treating physician/specialist and submit to Transplant Australia prior to the 9 September 2026 to be eligible to participant in any sporting competition
Instructions:
- Download and print a copy of the above medical form.
- Ask your physician / specialist to complete the medical form.
- Submit your form to Transplant Australia (submission link to come)
- Submit before the Wednesday, 9 September 2026
Transplant recipient competitors are to have their medical forms filled in by their treating physician between 8th May, 2026 and 2nd September, 2026.
In the event medical clearance cannot be obtained by the cutoff date, please contact Transplant Australia directly (as early as possible) to discussion your options via contactus@transplant.org.au.
For the 2026 Australian Transplant Games, living donors are no longer required to submit medical clearances but are strongly suggested to discuss their suitability to participate in the games with their treating physicians and prepare appropriately.
