Transplant Australia’s Medical Committee will manage the needs of athletes and their families at the Games.

Medical Forms

Download and print a copy of the medical form.
Ask your doctor to complete the medical form.
Submit your form HERE (You are required to enter all the information provided in the form)
Submit before the 9 September 2024.

Transplant recipient and living donor competitors are to have their medical forms filled in by their treating physician between 8th May, 2024 and 2^nd September, 2024.

Completed Medical Forms must then be uploaded by 9 September 2024.
